Please also fill out both the I-9 and W-2 forms below. Both forms can be either printed, filled out, and returned to our office, or filled out electronically and submitted to [email protected]. If any/all forms are being printed, please be sure to have all documents completely filled out and signed with any necessary documentation specified in the documentswhen you submit them to our office in person.
